黑色素瘤(melanoma)

黑色素瘤屬於皮膚癌中最嚴重的一種惡性腫瘤，一般來說其呈現的形式有三種，包括平面、鼓起（形成結節）、傷口（潰瘍）等，黑色素瘤的發生率和發生的形式與人種有關，一般來說，白種人的發生率遠高於黃種人和黑人，而黃種人好發的肢端型黑色素瘤在白種人中卻是發生率做低的
一、分類
A. 表淺擴散型(superficial spreading melanoma)：
a. 大約占70%
b. 常見於外國人，特別容易出現在曬太陽的部位
c. 應盡早切除
B. 結節型(Nodular melanoma)：
a. 大約占15%
b. 大多垂直生長
c. 容易被誤認為腫瘤，切掉後才發現是黑色素瘤
C. 肢端痣樣型(Acral lentiginous melanoma)：
a. 大約占8%
b. 皮膚黑一點的比較常見，是台灣人最常見的一種
c. 出現在沒有曬太陽的部位，例如：腳趾縫、腳底、背部等等。
d. 通常不痛不癢易被忽略
D. 惡性痣型(Lentigo maligna melanoma)：
a. 常發生在老年人
b. 大型表淺擴散的黑色素瘤(放射狀生長)

二、檢驗
 s100：非專一性(non-specific)，用來篩檢黑色素瘤，也可染神經物質的腫瘤
 HMB45：專一性(specific)，也可染淋巴腺轉移
 melan A：專一性(specific)
